About the Role
The Harm Reduction Navigator plays a vital role in providing comprehensive case management services to clients with substance use, mental health, and addiction needs. You will collaborate closely with clients to support care plans, facilitating access to various services such as counselling, therapy, and social support. In this position support. In this position, you will ensure seamless, coordinated care by working alongside healthcare professionals, community organizations, and external agencies.
This position plays a key role in supporting the community-based substance use program, a phased program and focuses on engagement, stabilization, skills development, and transition.
What you will be doing
This position reports directly to the Supervisor, Special Projects.
- Assessment: Conduct comprehensive assessments of clients’ substance use, mental health, and addiction needs, considering their unique circumstances, strengths, and challenges.
- Care Planning: Collaborate with clients to support services that address their substance use, mental health, and addiction goals. Monitor progress and adjust care plans as needed throughout the program’s phases.
- Service Coordination: Facilitate access to a range of services, including counseling, therapy, medication management, and social support, to meet clients’ needs.
- Advocacy: Advocate for clients’ rights and needs within the healthcare system, ensuring they receive quality care and support. Assist clients in accessing transitional supports, including housing stabilization, employment services, and community wellness programs.
- Collaboration: Work closely with other healthcare professionals, community organizations, and agencies to ensure seamless and coordinated care for clients. Collaborate with community partners, such as AA (Alcoholics Anonymous), NA (Narcotics Anonymous), and other support services, to enhance clients’ recovery journey.
- Documentation: Maintain accurate and up-to-date client records, documenting assessments, care plans, progress, and outcomes. Provide bi-weekly reports to the Harm Reduction Supervisor on program outcomes and client needs.
- Cultural Competence: Demonstrate cultural sensitivity and competence, recognizing the diverse backgrounds and needs of clients within the Metis community.
- Liaison: Liaise with other MNO at community events and activities related to mental health and addictions.
- Project Work: Implement annual project work plans and activity reports in accordance with program goals.
- Record Keeping: Maintain accurate and up-to-date records of all program-related activities.
- Reporting: Provide bi-weekly reports to the Supervisor of Mental Health and Addictions, detailing client progress, program outcomes, and service needs.
- Privacy and Confidentiality: Uphold strict standards of privacy and confidentiality in compliance with guidelines, organizational policies, and applicable privacy laws.
- Other reasonable duties as may be assigned.
Qualifications
- Strong desire to act in the service of the Metis Citizens, to advance and uphold rights and culture
- Completed post-secondary degree in a related field (Social Work, Addictions, or Mental Health) or a minimum of 2 years of experience in mental health and addictions, with a focus on harm reduction.
- Knowledge of harm reduction strategies and experience working with Indigenous or Metis populations is an asset.
- Computer Proficiency: Competency in using Microsoft Office programs (Outlook, Excel, Word, PowerPoint) and client health care management software to track client interactions and service delivery.
- Communication: Excellent oral and written communication skills, with the ability to convey information and advocate for clients.
- Organization and Time Management: Strong organizational skills with the ability to manage multiple tasks, prioritize work, and meet deadlines.
- Self-motivation: Demonstrated ability to work independently with minimal supervision, as well as collaboratively in a team environment.
- Problem-solving and Advocacy: Ability to resolve client issues, advocate effectively for their needs, and coordinate care across multiple systems.
- Demonstrated ability to work alone/independently with minimal supervision.
Preferred
- Completion of a Master’s degree in a relevant field
- Knowledge of Metis-specific and broader indigenous policy and research, key challenges, and programs
Experience
- Proven experience working in the field of adult mental health and addictions, with a strong understanding of harm reduction approaches.
- Experience and knowledge of Indigenous issues, particularly those affecting Metis peoples, are considered an asset.
- Strong knowledge of Ontario’s mental health and addiction programs and services, including best practices in care coordination and client advocacy.
Preferred
- Previous 2 years or more experience in a similar/comparable role
- Demonstrated ability to write high quality evidence and data-based reports and professional presentations
- Demonstrated ability in policy and program development and evaluation
- Demonstrated ability to quickly acquire and expand knowledge and to be flexible in a demanding and dynamic environment
- Previous experience dealing with confidential information
